Telemedicine is an at home monitoring system designed for the management of patients with Heart Failure/ Hypertension/ Heart Disease/ Diabetes/ Post Cardiac Surgery. Telemedicine utilizes Blue Tooth Technology along with the patient’s standard phone line to transmit vital clinical data to a secure HIPPA server. The information is remotely accessed via Internet connection by VNA and used to make objective, clinically sound decisions regarding the patient’s care.
